Colorado Diabetes Network: Diabetes, Hypertension and Chronic Kidney Disease - 21 May 2008

We are very excited to announce our upcoming Colorado Diabetes Network annual meeting focusing on Diabetes, Hypertension and Chronic Kidney Disease on May 21, 2008!
Because this meeting addresses three overlapping chronic diseases, we would like to invite individuals outside the diabetes network, to include those working with heart disease and chronic kidney disease. At this meeting we will provide basic information on:
- diabetes,
- hypertension and
- chronic kidney disease,
- including clinical guidelines and how to translate these guidelines into practice.
We will present tools to increase effective communication and we will address racial and ethnic disparities affecting these chronic diseases.
